DR 8100 S yang di kemudikan Alwan Wijaya membawa muatan berupa kayu kebun dan 12 orang penumpang/buruh yang duduk di bak belakang kendaraan melaju dari arah Dusun Munder menuju ke arah Desa Mumbul Sari (selatan ke utara) dan selanjutnya dalam perjalanan sebelum terjadinya kecelakaan, pengemudi merasakan ada masalah dengan rem kendaraannya dan sempat berhenti di sebuah bengkel di Dusun Belencong dekat tempat kejadian, pada saat di cek ternyata minyak rem kendaraannya habis, namun karena di bengkel tersebut tidak ada menjual minyak rem sehingga pengemudi melanjutkan perjalanannya dan setelah beberapa saat kendaraan tersebut berjalan melaju meninggalkan bengkel tersebut,
“tiba-tiba rem kendaraan tidak berfungsi atau blong membuat pengemudi tidak bisa menguasai laju kendaraannnya sehingga beberapa orang penumpang/buruh yang ada di bak kendaraan panik dan melompat dari atas kendaraan. Selanjutnya sekitar satu kilometer(km) dari penumpang loncat dari bak truk dan terjadilah kecelakaan tunggal” jelas Kasat Lantas.
